At the beginning, we see that the death of Harvey Dent was to cover up for Two-Face being put in Arkham. However, he is dying and is in a hospital at the moment. This is the case or Two-Face escaped months ago and is secretly hiding out in the Gotham Underworld but GCPD can't tell this to Gotham since Harvey is supposedly dead. Or of course, Two-Face actually died in TDK...
Later on, we hear mentions of crime bosses such as Rupert Thorne, Tony Zucco, and Alberto Falcone (JGL) who's taking over the underworld. Working under the mob is Bane (Tom Hardy), a bounty hunter. Years ago, he worked for the League of Shadows and was the body guard of Ra's Al Ghul who trained him and gave him knowledge of an "operation involving venom." Either overpowering him or being betrayed by Ra's, Bane left to be hired by the mob; particularly Thorne.
Now, Bane has been hired under the table to kill Batman once again but not by the mob... By GCPD's corrupt cop Harvey Bullock (or the one cop who didn't die that acts like Bullock from TDK).
Meanwhile, Batman is still on the run from police (taking crime into his own hands more than ever before and being blamed with the murder of Harvey Dent). Bruce Wayne has met Selina Kyle (Anne Hathaway). Taking a chance since he seems to have nothing left to loose (Rachel being gone and now being hunted by the cops), Bruce shows her his true side: Batman but without the cape and cowl.
Selina however is hiding something... Her much darker past than Bruce (arguable). Firstly, she went to a fundraiser for Wayne Enterprises to steal from him but after meeting Bruce, she didn't (or she did and this stolen jewelry is returned at the end of the film). These are her motives...
After running away from home as a teenager due to abuse years ago, she became associated with the mob and Rupert Thorne became a father to her. Growing up in the environment, she wants to pay Thorne back for everything he's done to help her.
She steals for Thorne from other mobsters' mansions, wearing black kevlar as the Batman does and a black mask also like him, with ears and orange athletic goggles, to look similar enough to just be thought of as a copycat when seen, an inspiration to her "work." She is joked about being a cat burglar among the underworld.
Selina currently lives in an apartment with Holly Robinson (Juno Temple) on the most crime populated side/poor part of the city (motives: stay close to Thorne or doesn't have enough money to move elsewhere).
By the end of the film, Two-Face passes away (if he wasn't already dead in TDK).
Batman sacrifices his life for Gotham, the city who hates him.
Selina Kyle avenges the legacy of the Batman as she becomes the new vigilante of the city.
A scene of Jim Gordon or/and a few main members of the GCPD, looking up into the sky at a custom bat signal made by Gothamites to remember the loss. Jim speaks about how Batman did what he did for the city.
Selina returns the stolen jewelry to Wayne Manor.
Alfred turns off the lights of the cave (loose the bat in front of it, I think this name would work best for the Nolanverse).
Fade out.
Bruce Wayne walks toward us, through paparazzi, cameras snapping pictures. He isn't dead. We hear questions from reporters in the loud environment, particularly one who says her name is Vicki Vale about "Are the rumors about adopting the child from the Flying Graysons (*the family of Batman's sidekick who's family was killed in a circus-like performance set up by mobster Tony Zucco, Robin, who would assumably take over the position of the Batman eventually) true?"
